Access and Use Agreement
for UW Data and Information Systems
(“Agreement (TEST)”)

As a UW Workforce Member, you are receiving this agreement because you, or someone on your behalf, requested that you have access to UW Information Systems or Institutional Information.

This Agreement, between the UW Workforce Member and the University of Washington, is intended to help safeguard and establish controls for access to and use of Institutional Information.

Underlined and linked terms found in this Agreement can be found in Administrative Policy Statements 2.4 Information Security and Privacy Roles, Responsibilities and Definitions and # Data Management Policy. For example, UW Workforce Members, defined in APS 2.4, are employees, trainees, students, volunteers, and other entities or persons who perform work for the UW through employment, academic or service agreements, or contracts. UW Workforce Members are required to follow all information security and privacy Policies, Standards, or Guidelines established by the UW or one of its organizational areas.

The Institutional Information is the property of the UW. UW Workforce Members access, use and disclosure of Institutional Information regardless of electronic device or format (e.g. verbal; raw data in SQL server objects such as databases and tables; reports; printed hardcopies) is to fulfill your UW job duties or to fulfill the service that you are performing for the UW.

As a UW Workforce Member and a user of UW Information Systems and Institutional Information, you have the following responsibilities:

  1. Protect your user name and password(s) to any UW Information Systems and Institutional Information for which you are granted access.
  2. Not to share your user name and password with other individuals or log into Information Systems for other individuals.
  3. Access and use UW Information Systems and Institutional Information to perform your authorized job duties or activities for UW.
  4. Accept accountability for all activities associated with the use of your user name, password, and related access privileges.
  5. When possible, use tools and common reports that are developed and implemented centrally in order to help ensure a consistent and common baseline for information at the UW.
  6. Physically protect hardcopies, and destroy or properly archive all printed or electronic materials as soon as their useful purpose has been served.
  7. Maintain the security of UW Information Systems for which you are granted access and/or authorized to use
  8. Safeguard the confidentiality of Confidential Information and protect Restricted Information accessed or obtained in the performance of your job duties or activities.
  9. Disclose Institutional Information (either internal within the University or external to other government agencies, vendors or the public) only to fulfill a legitimate business need as defined by managements direction and as authorized by the relevant Data Custodians or Trustees.
  10. Coordinate all public records requests with the UW Office of Public Records.
  11. Comply with UW policies and standards, including, but not limited to:

    • 2.5 Information Security and Privacy Incident Management Policy
    • 2.10 Minimum Data Security Standards
    • 57.09 Release of University Records

  12. Support compliance with laws and regulations related to information systems and institutional information, such as the Federal Family Educational Rights and Privacy Act of 1974 (FERPA).
  13. Report all Information Security Privacy Incidents, including the suspected misuse Institutional Information, as required in 2.5 Information Security and Privacy Incident Management Policy.

Enforcement

Failure to comply with this Agreement or the UW policies and standards may result in disciplinary action up to and including termination for UW employees, contract termination in the case of contractors or consultants; dismissal for interns and volunteers; or suspension or expulsion in the case of a student.

The UW reserves the right to pursue appropriate legal actions to recover any financial losses suffered as the result of a violation of this Agreement or UW policies and standards.
