UW NetID Identity Verification

You must specify a registration ID of the form: UWdddddd!

In order to sign up for a UW NetID you must first verify your identity.

Before you start you'll need:

  • A photo ID
  • A printer

This process entails:

  1. Use this process to fill out and print a registration form
  2. Have the form notarized by a licensed Notary Public (optional)
  3. Fax or mail the form and a photocopy of your photo ID to the UW-IT Service Center
  4. Once the registration form is processed:
    • If the form is notarized you will receive an email with UW NetID creation instructions.
    • If the form is not notarized then you will be sent UW NetID creation instructions via postal mail.
